Re-Code Report

Once you receive the results from the Mask Assessment, have them complete the Re-Code Report. Upon completion, you will receive two emails with reverse codes, which you'll need to combine.

Life Reflection Form

You will also need to fill out a Life Reflection online form, providing a snapshot of your life before the re-code process. You will receive a copy of this document for reference.
